The State of Illinois requires the schools to have specific information on file. The first item is a birth certificate for children entering kindergarten. You may get this from the courthouse. Parents of children born in Peoria County also have the option of ordering them online at: http://www.illinoisepay.com/epay/GetSite?cmd=get&siteId=204

They also require schools to have current forms on Health, Dental and Vision exams on each of student. The following list indicates which years we need a new form in the office. At the bottom of this page, we have provided links to forms you can download and take to your healthcare providers.

Kindergarten: Health/Immunization, Dental and Vision

2nd Grade: Dental

6th Grade: Health/Immunization, Dental and Vision

New enrollees: New Vision exams are required upon entering Concordia, unless the student has one less than a year old.

Extra-Curricular Sports: Physicals for new Health/Immunization forms are required every other year to participate in extra-curricular sports.
