Mission Statement

Concordia Lutheran School provides families with a Christ-centered, academically challenging education to equip students for lifelong Christian discipleship and serves as a witness of Christ to our community and world.

The Livingston Legacy Scholarship will provide  financial support to a selected student for an on-going Christian education at Concordia Lutheran School through the eighth grade. Concordia Lutheran School provides a Christ-centered, academically challenging education to equip students for lifelong discipleship.

The State of Illinois requires the schools to have specific information on file. The first item is a birth certificate for children entering kindergarten. You may get this from the courthouse. Parents of children born in Peoria County also have the option of ordering them online at:

They also require schools to have current forms on Health, Dental and Vision exams on file for each student. The following list indicates which years we need a new form in the office. 

Kindergarten: Health/Immunization, Dental and Vision

2nd Grade: Dental

6th Grade: Health/Immunization, Dental and Vision

New enrollees: New Vision exams are required upon entering Concordia, unless the student has one less than a year old.

Extra-Curricular Sports: Sports physicals are required each year for participants in 5th through 8th grade in extra-curricular sports.
